If you have previously completed a Participation Request form, please Log In. Do not complete this form more than once. Multiple applications could delay your approval. If you are an established, safe Big Way formation jumper and a team player, please obtain a reference from one of the Captain's listed and submit your Participation Request Form. Minimum Requirements: Recommendation of an approved event Captain, a USPA C or D License or foreign equivalent (and a POPS/SOS number for POPS or SOS events). NOTE: Big Way events are by invitation only and not all Participation Requests are approved. IF you are selected to participate, you will receive an Invitation and event information with payment instructions by email from an Event Captain. Do not send a payment until notified by email of your acceptance by an Event Captain. You will be notified after an Event Captain's approval at which time your slot deposit will be due. | |
